Case Management

Operation Sickle Cell Logo

Statewide newborn screenings, referrals, and aggressive follow up efforts have resulted in a continual swelling of the case management load at Community Health Interventions and Sickle Cell Agency, Inc. CHI SCA offers Case Management to clients who have Sickle Cell Disease.

Often times, case management consists of annual or biannual assessments of client health, home visits, child service coordination, referrals to medical and social agencies, and interventions on behalf of the client. CHI SCA strives to improve the quality of life for all of its clients and works hard to ensure that all clients are educated on the complications of sickle cell, home remedies, when to go to the doctor or emergency room, hereditary factors of the disease, and the importance of maintaining regular visits with a primary doctor and a hematologist. Additionally, CHI SCA staff stresses the importance of hydration, good nutrition, accurate medication administration, activity, and rest in order to maintain good health.
